Policy/Procedure on Athletic Pre Participation Physicals
1. All students registering for any MJSAO sports/team must be medically cleared for participation as per NJ State mandate.
2. Students will have completed a standard Athletic Pre-Participation Physical Evaluation History Form completed by a parent/guardian, an Athletes With Disabilities Supplement Form if needed, a Physical Examination Form completed by the Physician, and a Medical Eligibility Form completed by the Physician. Only the Medical Eligibility Form is uploaded on rSchoolToday. The Physician keeps the History and Physical Forms on file.
3. Athletic Medical Eligibility forms will be reviewed by a Registered Nurse and deemed complete.
4. Any form that is deemed incomplete will be returned to the parent/guardian so that corrections can be made and the form resubmitted. The RN will request any extra information or clearance letters if needed.
5. After the RN deems an Eligibility Form complete, the Nurse submits the form to the Board of Education hired Physician for final approval and signature on rSchoolToday.
6. If a student already has an approved Eligibility Form on file from a previous sports season that documents a physical exam that is within 365 days of the first tryout date, then the parent need only complete a Health History Update Form on rSchoolToday during the registration process. All “yes” checks need to be explained in detail.
7. The RN maintains a database for all submitted forms listing: Student Name, DOB, grade, date of physical exam, date of medical clearance, and date of any Health History Updates being approved in rSchoolToday.
8. The Parent/Guardian will inform Coaches of the student's medical conditions.
