Here is some helpful information about the registration process for athletics at MJS.

Be sure you and your physician use the correct athletic forms:
Athletic Forms
Only the last page, the Medical Eligibility Form gets turned into school.
The school nurse will contact you if the district physician needs a clearance letter from a specialist, or an extra note from your physician before final clearance is granted. You may already have this note if your child had to be excused from gym class for something like a recent orthopedic injury or concussion. An extra note may be needed if your physician checks off anything but “medically eligible for all sports without restrictions.”
The Eligibility Form must be dropped off to the nurse’s office at least 2 weeks prior to the first tryout.
Athletic forms are valid for 1 year from the date of examination, not the day the physician completes and signs the forms. For example, if your child had a physical examination August 15, 2025, the physical expires August 14th 2026.
The Health history update questionnaire must be submitted for each season of participation if your physical on file is dated more than 90 days from the start of the season.

Doctors offices can take up to 2 weeks to complete athletic forms so please plan accordingly.
The Eligibility Form is due in the nurse’s office 2 weeks before the first try out. This allows time for the district physician to review the form and grant final approval.
All students must receive final clearance from the district physician before trying out for a sport. There are no exceptions to this policy.
Be sure to speak with your child’s coach about any medical conditions that your child has. The coach should have a copy of your child’s action plan and your child must have their inhaler, epipen, or diabetic supplies at all try outs, practices, and games.
