Here is some helpful information about the registration process and athletic forms for athletics at MJS.
Instructions for Registration:
  1. Please visit mjs-athletic-registration. This site has the registration link and important practice and game information.  
  2. Click the “register now” button on the homepage. It will open a new window asking you to create an account or sign in with the Arbiter program. 
  3. Create an account, fill out the application, and upload your child’s medical eligibility form. The eligibility form must be a PDF. Photographs of documents are not accepted.


Please note the date registration closes. If your child is not registered by the closing date, they may not participate in the sport. Please note the date medical eligibility forms need to be uploaded in Arbiter. This allows the district physician time to review all forms and clearance letters. Your child may not try out for a sport without final clearance from the district physician. There are no exceptions to this  policy.

You will be contacted if the district physician needs a clearance letter from a specialist, or an extra note from your physician before final clearance is granted. You may already have this note if your child had to be excused from gym class for something like a recent orthopedic injury or concussion.  

Athletic forms are valid for 1 year from the date of examination, not the day the physician completes and signs the forms. For example, if your child had a physical examination August 15, 2025, the physical expires August 14th 2026. 

The health history update questionnaire is required and this document is completed automatically in Arbiter during the registration process. All “yes” checks need to be explained in detail.

Doctors offices can take up to 2 weeks to complete athletic forms so please plan accordingly.
If athletic forms are needed quickly, places like CVS Minute Clinic complete forms on the spot.

If you are having difficulty uploading forms, try deleting any old forms. Arbiter does not allow the nurse to delete or upload any documents.

Please speak to your child’s coach about any medical conditions and your child has their inhaler, epipen, or diabetic supplies at all try outs, practices, and games.
